Diagnostic Imaging for Baxter Nerve Compression

Diagnostic Recommendations

  • The American College of Radiology recommends MRI without IV contrast as the preferred imaging study after negative or equivocal radiographs for diagnosing Baxter nerve compression, with key findings including denervation changes in the abductor digiti minimi muscle 1
  • Initial imaging should begin with radiography of the foot for chronic foot pain of unknown etiology, followed by MRI foot without IV contrast as the recommended next imaging study after negative or equivocal radiographs 1

Treatment Guidelines

  • The American College of Radiology and other medical societies suggest that patients with Baxter's neuropathy may benefit from conservative management, including physical therapy with neural gliding exercises and core strengthening, as well as pharmacological management with NSAIDs as first-line therapy for pain management 2

Treatment for Baxter Nerve Compression

Important Clinical Considerations

  • The American Diabetes Association recommends avoiding rushing to surgical decompression, as the evidence does not support nerve decompression procedures for prevention or routine treatment, given the inherent surgical risks and availability of effective conservative options 3
