Rizatriptan Dosing Considerations for Migraine
Cardiovascular Disease Considerations
- The American Academy of Family Physicians recommends that rizatriptan is contraindicated in patients with ischemic vascular conditions, vasospastic coronary disease, uncontrolled hypertension, or other significant cardiovascular disease 1, 2
- Triptans can cause coronary vasospasm and should not be used in patients with coronary artery disease (CAD), according to the American Family Physician 1
- Alternative agents such as gepants (ubrogepant, rimegepant) should be considered for patients with cardiovascular contraindications, as suggested by the Praxis Medical Insights 3
Medication Overuse Headache Prevention
- The Praxis Medical Insights recommends limiting rizatriptan use to no more than 2 days per week to prevent medication-overuse headache, and not treating more than 4 headaches per 30-day period on average 3
- Medication-overuse headache can result from frequent use of acute medications (more than twice weekly), leading to increasing headache frequency and potentially daily headaches, according to the Praxis Medical Insights 3
- If patients require acute treatment more than twice weekly, preventive therapy should be initiated immediately, as recommended by the Praxis Medical Insights 3
