Management of Mania with Insomnia in Bipolar Patients
Recommended Medication Changes
- The American Academy of Sleep Medicine recommends against using trazodone for both sleep onset and sleep maintenance insomnia 1, 2
- Replace trazodone with a short-acting benzodiazepine receptor agonist such as zolpidem 10mg at bedtime or eszopiclone 2-3mg at bedtime, which have stronger evidence for efficacy in insomnia and less risk of inducing mania 1, 2, 3
- If mania persists, consider adding an atypical antipsychotic with sedating properties, such as quetiapine or olanzapine, to help with both mania and sleep 4
Monitoring and Follow-up
- Monitor for sedation, dizziness, and psychomotor impairment, especially with the addition of a benzodiazepine receptor agonist 3
- Reassess sleep quality using patient self-report of sleep latency, maintenance, and quality 3
Important Cautions
- Benzodiazepine receptor agonists should be used at the lowest effective dose and for the shortest possible duration 1, 2
- Patients should be counseled about potential risks including sleep behaviors (sleepwalking, sleep driving) associated with sedative medications 2
- Regular follow-up is essential to assess medication effectiveness, side effects, and the ongoing need for pharmacotherapy 2
- Once the acute manic episode resolves, consider transitioning to non-pharmacological approaches for insomnia such as cognitive behavioral therapy for insomnia (CBT-I) 1, 2
