Diabetic Medications for Patients with End-Stage Renal Disease

First-Line Medications for ESRD

  • The American Journal of Kidney Diseases recommends insulin as the cornerstone of diabetes management in ESRD, with careful dose adjustments due to decreased renal clearance 3, 4
  • Patients with ESRD require conservative insulin dosing with careful titration to avoid hypoglycemia, as decreased renal clearance prolongs insulin half-life 3
  • The Kidney International guideline suggests that several GLP-1 receptor agonists, including dulaglutide, liraglutide, and semaglutide, can be used in ESRD without dose adjustment 5
  • The Annals of Internal Medicine recommends DPP-4 inhibitors, such as linagliptin, as a first-line option for patients with ESRD, with minimal hypoglycemia risk 2, 6

Medications to Avoid or Use with Caution in ESRD

  • The American Journal of Kidney Diseases states that metformin is contraindicated in ESRD due to the risk of lactic acidosis, and should be discontinued when eGFR falls below 30 ml/min/1.73m² 1, 4
  • The Kidney International guideline recommends avoiding SGLT2 inhibitors in ESRD with eGFR <15 ml/min/1.73m², except for certain agents like dapagliflozin and canagliflozin, which may be continued until dialysis for cardiovascular and kidney benefits 5
  • The American Journal of Kidney Diseases advises against using first-generation sulfonylureas in ESRD, and recommends using second-generation sulfonylureas with extreme caution due to significant hypoglycemia risk 3, 5

Special Considerations

  • The Annals of Internal Medicine suggests that medication choices should consider comorbidities like heart failure and atherosclerotic cardiovascular disease, and recommends GLP-1 RAs with proven cardiovascular benefits for patients with cardiovascular disease 2, 6
  • The Kidney International guideline recommends considering DPP-4 inhibitors, especially linagliptin, or GLP-1 RAs, such as dulaglutide, liraglutide, and semaglutide, for patients with concerns about hypoglycemia 5
