Therapeutic Thoracentesis and Pleural Management in Pancreaticopleural Fistula

Indications for Thoracentesis

  • Perform therapeutic thoracentesis only in patients who are symptomatic with dyspnea; asymptomatic pleural effusions should not be drained. Ultrasound guidance is recommended for all pleural interventions. 1, 2, 3
  • Large‑volume thoracentesis may be performed in symptomatic patients to evaluate relief of dyspnea and to aid diagnosis. 1

Imaging Guidance for Pleural Procedures

  • Use ultrasound guidance for every pleural intervention to improve safety and procedural success. 1

Management of Recurrent or Persistent Effusions

  • In cases of recurrent symptomatic effusions, placement of an indwelling pleural catheter (IPC) is preferred over repeated therapeutic thoracentesis. 1

Avoidance of Unnecessary Drainage

  • Do not perform pleural interventions (including thoracentesis or drainage) in asymptomatic patients, even when a pancreaticopleural fistula is suspected, to avoid unnecessary risk. 1, 2, 3
  • Do not drain asymptomatic fluid collections, as this can introduce infection without clinical benefit. 2, 3
