Valacyclovir Treatment for Oral Herpes

Efficacy and Safety

  • Short-course valacyclovir therapy has been shown to significantly reduce the duration of cold sore episodes compared to placebo, with a median duration of episodes of 4.0-5.0 days compared to 5.0 days for placebo (p<0.001) 1, 2
  • Valacyclovir has been shown to accelerate healing and decrease pain associated with herpes labialis 3
  • Early initiation of therapy is critical, with treatment beginning during the prodromal phase or within 24 hours of symptom onset for best results 3, 4
  • Valacyclovir is generally well-tolerated with minimal adverse events, with the most common side effects including headache, nausea, and diarrhea 4, 5

Special Considerations

  • Dose adjustment may be required in patients with significant renal impairment 6
  • For immunocompromised patients, particularly those with HIV infection, higher doses or longer treatment durations may be required 6, 7
  • Resistance to valacyclovir remains low (<0.5%) in immunocompetent hosts but may be higher in immunocompromised patients 5

Comparison to Other Treatments

  • Famciclovir is also effective for herpes labialis, with single-dose regimens (1500mg) showing significant reduction in time to healing compared to placebo 8
  • Both valacyclovir and famciclovir offer more convenient dosing schedules than acyclovir for herpes labialis 1, 2, 8

Valacyclovir Treatment for Herpes Labialis

Special Considerations

  • Athletes with a history of recurrent herpes labialis should be considered for suppressive antiviral therapy rather than episodic treatment, as recommended by the Pediatrics guideline society 9
  • Topical antivirals should not be used as primary therapy for herpes labialis, as they are substantially less effective than systemic therapy, according to the Pediatrics guideline society 9
