Semaglutide Dosing and Administration Guidelines for Chronic Weight Management

Introduction to Semaglutide Titration

  • The American College of Cardiology and other guideline societies recommend initiating semaglutide at 0.25 mg subcutaneously once weekly and titrating upward every 4 weeks through a fixed escalation schedule to minimize gastrointestinal side effects 1

Standard Titration Schedule

  • The standard titration schedule for semaglutide follows this specific timeline: Weeks 1-4 at 0.25 mg, Weeks 5-8 at 0.5 mg, Weeks 9-12 at 1.0 mg, Weeks 13-16 at 1.7 mg, and Week 17 onward at 2.4 mg (maintenance dose) 1, 2

Administration Practicalities

  • Administer semaglutide at any time of day, with or without meals, and store refrigerated at 36°F to 46°F (2°C to 8°C) or at room temperature for up to 28 days if needed 1

Missed Dose Management

  • If a dose is missed, administer as soon as possible if less than 5 days have passed, and resume the regular weekly schedule; if more than 5 days have passed, skip the missed dose and give the next dose on the regularly scheduled day 1

Dose Optimization Considerations

  • Not all patients require the maximum 2.4 mg dose, as some achieve significant weight loss at submaximal doses (1.0 mg or 1.7 mg) and can continue long-term at that dose if losing ≥1 lb per week and tolerating the medication well 1

Special Populations

  • No dosage adjustment is required for patients with renal impairment, even in end-stage renal disease, which is a significant practical advantage over many other medications 1, 3
  • Patients with type 2 diabetes may require dose reductions of insulin or sulfonylureas when combining with semaglutide due to hypoglycemia risk 1, 3

Critical Drug Interactions and Contraindications

  • Do not combine semaglutide with other GLP-1 receptor agonists or DPP-4 inhibitors due to increased risk of adverse effects 1, 4
  • Absolute contraindications include personal or family history of medullary thyroid carcinoma, multiple endocrine neoplasia syndrome type 2 (MEN2), pregnancy, and breastfeeding 1, 2, 5

Monitoring Requirements

  • Monitor patients for signs and symptoms of pancreatitis, acute gallbladder disease, suicidal ideation and behavior changes, and hypoglycemia if on concurrent insulin or sulfonylureas 1, 2, 4, 6

Efficacy Expectations and Discontinuation Criteria

  • Clinical judgment should guide continuation of semaglutide therapy; if patients show no meaningful response after reaching maintenance dose for 12-16 weeks, consider alternative therapies 2, 4, 5

Oral Contraceptive Interaction

  • Women of reproductive age should use a barrier method for 4 weeks after initiation and after each dose increase of semaglutide due to decreased effectiveness of oral contraceptives 6
