Anterior Mediastinal Tumors
Epidemiology and Prevalence
- Thymomas are rare tumors with an incidence of approximately 1.5-2 cases per million per year in the United States 1, 3
- Thymomas represent the most common primary neoplasm of the anterior mediastinum, accounting for approximately 50% of all anterior mediastinal tumors in adults 1, 2, 3
Differential Diagnosis of Anterior Mediastinal Masses
- The National Comprehensive Cancer Network recommends considering neoplastic conditions such as thymoma, lymphoma, thymic carcinoma, teratoma, and other germ cell tumors in the differential diagnosis of anterior mediastinal masses 2
- Non-neoplastic conditions like intrathoracic/retrosternal goiter and thymic cysts should also be considered in the differential diagnosis 1, 2, 4
Clinical Presentation
- Patients with thymomas often present with an indolent course, whereas those with lymphoma or germ cell tumors typically have a more rapid onset of symptoms 4
- Thymomas are uniquely associated with paraneoplastic syndromes, particularly myasthenia gravis 3
Diagnostic Approach
- The National Comprehensive Cancer Network recommends CT chest with contrast as the primary imaging modality for evaluating anterior mediastinal masses 4
- Serum markers including beta-HCG and AFP help differentiate between various anterior mediastinal tumors 4
