Low Back Pain Management

Initial Assessment and Red Flags

  • The American College of Physicians recommends focusing on conservative treatment without imaging for patients with low back pain unless red flags are present, such as progressive neurological deficits, bladder/bowel dysfunction, history of cancer, unexplained weight loss, fever or signs of infection, severe trauma, or age >50, which has a modest risk factor for cancer with a positive likelihood ratio of 2.7 1
  • Red flags for low back pain include bladder/bowel dysfunction, with urinary retention having 90% sensitivity for cauda equina syndrome, and other signs such as fever, severe trauma, or age >50 1

Physical Examination and Diagnostic Tests

  • The straight-leg raise test has 91% sensitivity but only 26% specificity for herniated disc, while the crossed straight-leg raise has 29% sensitivity but 88% specificity 1
  • A neurological examination should focus on assessing nerve root function, including knee strength and reflexes for L4, great toe and foot dorsiflexion strength for L5, and foot plantarflexion and ankle reflexes for S1, as well as sensory distribution of symptoms 1
  • The following table compares the sensitivity and specificity of MRI and CT for lumbar disc herniation:
Modality Sensitivity Specificity
MRI 91.7% 100%
CT 83.3% 71.4%

2

  • MRI is strongly recommended for patients with persistent low back pain and signs or symptoms of radiculopathy or spinal stenosis, particularly when they are potential candidates for surgery or epidural steroid injection, as per the American College of Physicians and American Pain Society guidelines 1
  • Lumbar radiography has limited utility for evaluating soft tissue causes of radiculopathy and spinal stenosis, and cannot visualize neural compression 1
  • Imaging is not recommended for uncomplicated acute lower back pain (duration <4 weeks) 3
  • Consider imaging only if symptoms persist beyond 6 weeks despite conservative management, red flags are present, or progressive neurological deficits develop 3
  • The American College of Radiology recommends MRI without contrast as the first-line imaging study for patients with suspected disc herniation presenting with radicular symptoms 2, 4

Treatment and Management

  • Physical therapy is beneficial for many back pain conditions, but progressive neurological symptoms warrant diagnostic evaluation first to rule out serious pathology requiring surgical intervention 1
  • The American College of Physicians recommends NSAIDs (e.g., ibuprofen) as first-line treatment for pain relief in patients with low back pain 1
  • Acetaminophen can be used as an alternative or in combination with NSAIDs for pain relief in patients with low back pain 1
  • Systemic corticosteroids are not recommended for treatment of low back pain with or without sciatica 1
  • Patients with low back pain should begin with light intensity activities such as walking and gradually increase frequency and intensity of physical activity 5
  • A progression of physical activity is recommended for patients with low back pain:
  • The American College of Physicians recommends considering gabapentin for neuropathic pain with radiculopathy, and duloxetine as second-line therapy for chronic pain 6, 1, 7
  • Image guidance (fluoroscopy) is essential for the safety and efficacy of Transforaminal Epidural Steroid Injection (TFESI), as stated by the British Journal of Anaesthesia, with TFESI recommended for patients who have failed conservative management, with radicular symptoms consistent with imaging findings 8, 3
  • Spinal cord stimulation is appropriate for patients with moderate to severe chronic neuropathic pain with radiculopathy that has persisted for more than 12 months, according to the Mayo Clinic Proceedings 9
  • Surgery should only be considered when pain control is unsuccessful after adequate conservative management, motor deficit greater than grade 3 is present, radicular pain is associated with foraminal stenosis, or cauda equina syndrome is present, as recommended by the American College of Physicians, with lumbar discectomy without fusion as the surgical procedure of choice for routine disc herniations 1, 10

Prognosis and Follow-up

  • Factors that predict poorer outcomes for low back pain include depression, passive coping strategies, job dissatisfaction, higher disability levels, and disputed compensation claims 1
  • Patients with low back pain should be reevaluated if symptoms persist beyond 4 weeks, or earlier if severe pain or functional deficits develop, signs of radiculopathy develop, or if the patient is older 1
  • The recovery timeline can be categorized into phases, with the majority of patients experiencing significant improvement within 4-12 weeks, although individual factors such as severity of trauma, age, and presence of comorbidities can influence the recovery trajectory, as noted by the American College of Radiology 11
  • Regular clinical and functional assessments are recommended after surgical intervention, with radiographic evaluation at 12 months post-surgery, and monitor response to treatment and use validated assessment tools to track progress, with regular follow-up every 4-6 weeks initially, as recommended by general medical practice and the American College of Radiology 12, 13, 14
