Chronic Lymphocytic Leukemia Management
Initial Management and Treatment Criteria
- For early-stage asymptomatic CLL, the American College of Hematology recommends a "watch and wait" approach with monitoring every 3 months rather than initiating immediate treatment, as early intervention has not demonstrated survival benefit in asymptomatic patients 1, 2, 3
- Treatment should begin only when patients meet criteria for "active disease," including constitutional B symptoms, cytopenias, symptomatic lymphadenopathy, splenomegaly, hepatomegaly, progressive lymphocytosis, or autoimmune anemia/thrombocytopenia poorly responsive to corticosteroids 1, 2, 4
Diagnostic Testing and Prognosis
- Essential pre-treatment testing includes FISH testing for del(17p) and chromosomal abnormalities, TP53 mutation testing, IGHV mutational status, and fitness assessment, as these tests fundamentally alter treatment selection and prognosis 2, 3, 4
First-Line Treatment
- For patients without del(17p) or TP53 mutations, the National Comprehensive Cancer Network recommends venetoclax plus obinutuzumab for 12 months as the first choice for patients with mutated IGHV, and a second-generation BTK inhibitor for those with unmutated IGHV 4
- For patients with del(17p) or TP53 mutations, the European Society for Medical Oncology recommends second-generation BTK inhibitors as first-line therapy, administered continuously 3, 4
Treatment of Relapsed/Refractory Disease
- If relapse occurs >12-24 months after initial therapy, the first-line treatment may be repeated if it was well-tolerated and achieved a durable response 2, 4
- If relapse occurs <12 months or disease is refractory, switch to an alternative drug class, such as BTK inhibitor or venetoclax-based regimen 1, 4
Monitoring and Follow-Up
- Monitor patients during treatment with CBC with differential every 2-4 weeks during chemoimmunotherapy, and every 2-4 months during BTK inhibitor therapy, as well as physical examination for lymph node and spleen size at each visit 4
- Bone marrow biopsy should only be performed to confirm complete remission, not routinely 2, 4
