Risk of Serotonin Syndrome When Combining Linezolid with SSRIs

Mechanism of Interaction

  • Linezolid acts as a reversible, non‑selective monoamine oxidase inhibitor (MAOI) and can precipitate serotonin syndrome when co‑administered with serotonergic agents such as selective serotonin reuptake inhibitors (SSRIs)【1】【2】【3】【4】.

Clinical Presentation of Serotonin Syndrome

  • The syndrome manifests with altered mental status (e.g., confusion, agitation, anxiety), neuromuscular hyperactivity (tremor, clonus, hyperreflexia, rigidity), and autonomic instability (hypertension, tachycardia, arrhythmias, diaphoresis)【1】【2】.
  • Severe cases may progress to fever, seizures, life‑threatening arrhythmias, loss of consciousness, and can be fatal【1】【2】.

Onset Timing

  • Symptoms typically develop within 24–48 hours after simultaneous exposure to linezolid and an SSRI【1】【2】.

Contraindication Guidance

  • The American Thoracic Society (via the American Journal of Respiratory and Critical Care Medicine) advises that linezolid should generally be avoided in patients receiving any serotonergic medication because of the risk of serious central nervous system reactions【3】【4】.

Management of Suspected Serotonin Syndrome

  • Immediate discontinuation of all serotonergic drugs is required, followed by hospital‑based supportive care and continuous cardiac monitoring to address potential complications【1】【2】.
