Medication Interactions Causing Weakness

Introduction to Medication Interactions

  • The American Heart Association suggests that the combination of carbamazepine and losartan can cause weakness, mainly due to the interaction between the two medications, which reduces the efficacy of losartan and increases the risk of adverse effects 1

Psychotropic Medications and Weakness

  • The use of quetiapina (50mg at night) can cause dose-dependent sedation and muscle weakness as common adverse effects, especially at night 2
  • The American College of Cardiology recommends that the dose of 50mg (2 tablets of 25mg) may cause residual morning sleepiness and asthenia 1

Cardiovascular Medications

  • The combination of amlodipino, losartana, and rosuvastatina is generally safe and well-tolerated, with no significant pharmacokinetic interactions, according to the European Society of Cardiology 3
  • The amlodipino does not interact significantly with rosuvastatina, and no dose adjustment is required, as stated by the American College of Cardiology 3

Mechanism of Weakness

  • The failure of losartan therapy due to enzymatic induction by carbamazepine can result in uncontrolled hypertension, as reported by the American Heart Association 1
  • The sedative and myorelaxant effects of quetiapina can contribute to weakness 2
  • Fatigue related to sertralina can also contribute to weakness, according to the National Institute of Mental Health 1
  • Orthostatic hypotension caused by the combination of antihypertensives with psychotropics can lead to dizziness and weakness 2

Management Recommendations

  • Consider replacing carbamazepine with an alternative anticonvulsant that does not induce enzymes (such as levetiracetam, lamotrigina, or valproic acid), as recommended by the American Academy of Neurology 1
  • Monitor blood pressure in different positions to evaluate orthostatic hypotension, as suggested by the American College of Cardiology 2
  • If replacement of carbamazepine is not possible, consider increasing the dose of losartan or adding another antihypertensive not metabolized by CYP450, according to the European Society of Cardiology [1
  • Evaluate the need to reduce the dose of quetiapina if sedation is excessive, as recommended by the National Institute of Mental Health 2
  • Consider administering sertralina at night if morning fatigue is prominent, as suggested by the American Psychiatric Association 1

Monitoring

  • Check liver enzymes and CPK to rule out hepatotoxicity or myopathy caused by rosuvastatina, as recommended by the American College of Cardiology [5
  • Monitor electrolytes, especially potassium, due to the combination of losartana with other medications, as suggested by the European Society of Cardiology 2
  • Evaluate renal function, as multiple medications are eliminated renally, according to the National Kidney Foundation 5
