Empiric Treatment of Recurrent Bacterial Vaginosis with Metronidazole Gel

  • The Centers for Disease Control and Prevention recommends metronidazole gel 0.75%, one full applicator (5g) intravaginally twice daily for 5 days as first-line therapy 1, 2
  • The American College of Obstetricians and Gynecologists notes that single-dose oral metronidazole (2g) has lower efficacy (84% vs 95% for multi-day regimens) for recurrent cases 3

Important Counseling Points

  • Patients must avoid all alcohol consumption during treatment and for 24 hours after completion to prevent disulfiram-like reactions 1, 2
  • The gel is oil-based and may weaken latex condoms and diaphragms during the treatment period 2
  • Patients with known allergy to oral metronidazole should not use the vaginal formulation 2
  • Follow-up visits are unnecessary if symptoms resolve after treatment 1, 2

Common Pitfalls to Avoid

  • The Centers for Disease Control and Prevention states that routinely treating sex partners does not influence treatment response or reduce recurrence rates in clinical trials 1, 2, 3
  • Metronidazole gel should not be used for treating trichomoniasis, as topical preparations achieve <50% efficacy compared to oral therapy for that condition 4
