Hydromorphone Dosing in Renal Impairment

Dose Adjustments and Alternative Opioids

  • General guideline recommendations support dose reduction when eGFR <60 mL/min/1.73 m², according to the American Journal of Kidney Diseases 1
  • Fentanyl and buprenorphine are preferred alternatives for severe CKD (eGFR <30 mL/min) as they have no active metabolites and can be used safely, as recommended by the American Journal of Kidney Diseases and other guidelines 1, 2
  • Tramadol should be avoided in severe renal impairment (GFR <30 mL/min) and ESRD, according to Praxis Medical Insights 3
  • Institute prophylactic bowel regimen with stimulant or osmotic laxatives for all patients on sustained opioid therapy, as recommended by Praxis Medical Insights 2

Practical Dosing Algorithm

  • For eGFR <30 mL/min, start at 25% of standard dose or consider alternative opioid, such as fentanyl, buprenorphine, or methadone, as recommended by Praxis Medical Insights and the American Journal of Kidney Diseases 1, 2
  • Increase dosing intervals in addition to reducing individual doses, according to the American Journal of Kidney Diseases 1
