PGE2 Gel Induction Protocol

Dosing and Administration

  • The American Academy of Family Physicians recommends administering a single intravaginal dose of 0.5 mg intracervically or 2-5 mg intravaginally, with continuous fetal and uterine monitoring from 30 minutes to 2 hours post-administration, and remove or reassess at 12 hours 1, 2

Critical Monitoring Requirements

  • The American Academy of Family Physicians mandates continuous fetal heart rate and uterine activity monitoring from 30 minutes to 2 hours after PGE2 gel administration, and for vaginal inserts, monitoring must continue from insertion until at least 15 minutes after removal 1, 2
  • The American College of Obstetricians and Gynecologists recommends removing PGE2 insert immediately if uterine tachysystole or hyperstimulation occurs, or if fetal distress develops 1, 3

Timing Before Oxytocin

  • The American College of Obstetricians and Gynecologists recommends waiting at least 12 hours after completion of cervical ripening before considering cesarean delivery for "failed" induction in the latent phase, if cervical ripening is complete but labor has not begun after 12 hours 4, 5

Safety Considerations

  • The American Academy of Family Physicians reports that the most frequent complications (≥2%) include uterine tachysystole without fetal distress, uterine tachysystole with fetal distress, and fetal distress without uterine tachysystole 1
  • The American College of Obstetricians and Gynecologists notes that the uterine rupture risk with prostaglandin use in scarred uteri is 2% for PGE2, compared to 13% for misoprostol 3

Storage and Handling

  • The American Academy of Family Physicians recommends keeping CERVIDIL vaginal inserts frozen until use, and not warming them prior to insertion, while dinoprostone requires refrigeration to maintain potency 1, 2

Cost Considerations

  • The American Academy of Family Physicians reports that dinoprostone gel kits cost $65-75, while the CERVIDIL vaginal insert costs approximately $165, representing a significantly higher cost compared to misoprostol 1, 2
