Laser Hair Removal Guidelines

Introduction to Laser Types

  • The American Academy of Dermatology recommends the Nd:YAG laser at 1064 nm as the most consistently effective option for permanent hair follicle removal, with the strongest controlled trial evidence, particularly for achieving follicular destruction across all skin types 1
  • The Nd:YAG laser has the largest number of controlled trials and case reports showing consistently effective results for follicular destruction, with typical treatment parameters including a 10-mm spot size with 10-ms pulse duration and 35-50 J/cm² for Fitzpatrick skin types I-III 1, 2, 3
  • For darker skin types IV-VI, the recommended parameters for the Nd:YAG laser are 20-ms pulse duration with 25-40 J/cm² 1, 2
  • The clinical endpoint for Nd:YAG laser treatment is delayed post-treatment perifollicular erythema and/or edema indicating follicular destruction 1, 3
  • Most protocols require 3-4 treatment sessions with the Nd:YAG laser, though additional treatments may provide more lasting follicular reduction 1, 2, 3
  • In randomized controlled trials, the Nd:YAG laser achieved 65-73% reduction in follicular activity across anatomic sites 4, 5, 6

Laser Systems for Hair Removal

  • The Alexandrite laser at 755 nm is effective for hair removal with long-pulsed configurations, working through follicular destruction and anti-inflammatory effects similar to Nd:YAG 1, 2, 3
  • The recommended fluence for the Alexandrite laser is 20-40 J/cm² 7
  • The CO2 laser was the first used for follicular destruction and is employed for excision, marsupialization, and vaporization, with large retrospective series showing consistently positive outcomes in Hurley stage II-III disease 1, 2, 3
  • Recurrence rates for CO2 laser treatment range from 1.1% to 29% depending on technique and follow-up duration, with a mean healing time of 4 weeks when using secondary intention healing 4, 5, 6
  • Intense Pulsed Light (IPL) is a non-coherent light source with a wavelength range of 590-1200 nm, with clinical studies showing approximately 80% hair reduction after 3-5 sessions at 6-week intervals, and randomized trials demonstrating significant improvement maintained at 12 months 4, 5, 6

Treatment Considerations

  • The American Academy of Dermatology recommends proper patient selection and tailoring fluence to skin type as the most important factors for efficacy and tolerability of laser hair removal treatments 1
  • Darker skin types (Fitzpatrick IV-VI) require adjusted parameters to minimize complications, while lighter skin types (I-III) can use any laser system with appropriate fluence adjustments 1
