Risks of Sertraline

Critical Life-Threatening Risks

  • Serotonin syndrome is a potentially fatal complication most often occurring with simultaneous use of multiple serotonergic drugs, with common symptoms including clonus, tremor, hyperreflexia, agitation, mental status changes, diaphoresis, and fever, and treatment requires immediate cessation of all serotonergic agents, with benzodiazepines for short-term symptom management, according to the American Urological Association 1, 2
  • The FDA mandates a black box warning for increased suicidal thinking and behavior in patients through age 24 years, particularly during the first months of treatment and following dose adjustments, and close monitoring for suicidality is mandatory, especially early in treatment and with any dosage changes, as recommended by the FDA 4
  • A systematic review of 18,526 patients found no significant difference in suicidal ideation in adult men treated with antidepressants versus placebo, but a small increase in risk was noted in patients under age 18, according to the American Urological Association 1, 2
  • Never combine sertraline with MAOIs—this combination is potentially fatal, as stated by the medical community 3

Psychiatric and Neurological Risks

  • Treatment with SSRIs should be avoided in men with a history of bipolar depression due to risk of mania, according to the American Urological Association 1, 2
  • Sertraline is moderately activating but well tolerated compared to other SSRIs, as reported by Praxis Medical Insights 4

Serious Metabolic and Hematologic Risks

  • Bleeding risk is strongly dose-related and particularly concerning in patients with pre-existing platelet dysfunction, representing a relative contraindication in patients with bleeding disorders, as stated by Praxis Medical Insights 3

Special Population Considerations

  • Sertraline has a low potential for drug interactions at the cytochrome P450 enzyme level, offering advantages over paroxetine, fluoxetine, and fluvoxamine, and it may interact with drugs metabolized by CYP2D6, as reported by Praxis Medical Insights 5
