Management of Postpartum Depression with Fluoxetine
Assessment and Treatment Response
- The patient is showing positive response with reduction in intrusive thoughts after just one week of fluoxetine treatment, which is an encouraging early sign of effectiveness, and early reduction in symptom severity is a good predictor of treatment response at 12 weeks 1
- Maintain treatment for at least 12-24 months after achieving remission to prevent relapse 1
Monitoring and Adjustment
- Schedule a follow-up appointment in 2-4 weeks to assess ongoing response to treatment, and evaluate for any worsening of symptoms, emergence of suicidal ideation, or medication side effects 1
- Monitor for any signs of serotonin syndrome, particularly if other medications are added 1
- If the patient is not responding adequately after 8-12 weeks at an optimal dose, consider alternative treatment strategies 1
Combination Therapy
- Consider adding cognitive-behavioral therapy (CBT) if available, as the combination of fluoxetine and CBT has shown greater efficacy than medication alone 1
