Management of ESRD Patients with ANCA Vasculitis History Presenting with Hemoptysis
Immediate Recognition and Risk Assessment
- Hemoptysis in ESRD patients with ANCA vasculitis history represents life-threatening diffuse alveolar hemorrhage from disease relapse, requiring urgent treatment to prevent respiratory failure and death, as recommended by the Kidney Disease: Improving Global Outcomes (KDIGO) guidelines 1, 2
Immediate Treatment Protocol
- The Kidney Disease: Improving Global Outcomes (KDIGO) guidelines suggest plasmapheresis for patients with diffuse pulmonary hemorrhage (Grade 2C recommendation), which should be initiated within 24 hours of presentation 1, 2
- High-dose corticosteroids should be started immediately, with intravenous methylprednisolone 500-1000 mg daily for 3 days, followed by oral prednisone 1 mg/kg/day, as recommended by the European League Against Rheumatism (EULAR) 1, 2, 3, 4
- Either rituximab or cyclophosphamide should be chosen, with rituximab being the preferred option for relapsing AAV, based on the 2024 KDIGO guidelines 5, 1, 3
Essential Supportive Care and Prophylaxis
- Pneumocystis jiroveci prophylaxis is mandatory, with trimethoprim-sulfamethoxazole 800/160 mg on alternate days or 400/80 mg daily, as recommended by the American College of Rheumatology (ACR) 4, 7
Post-Acute Management
- Rituximab is preferred for maintenance in relapsing disease, with either the MAINRITSAN protocol (500 mg every 6 months) or the RITAZAREM protocol (1000 mg at months 4, 8, 12, 16), as recommended by the KDIGO guidelines 5
- Maintenance duration should be 18 months to 4 years after achieving remission, with ongoing monitoring for relapse, as recommended by the KDIGO guidelines 5
