Methylprednisolone Dosing in Pediatric Patients

General Dosing Principles

  • The American College of Rheumatology recommends first-line therapy typically being 1-2 mg/kg/day IV for most inflammatory conditions, while intensification therapy requires 10-30 mg/kg/day IV for refractory cases 1, 2
  • Methylprednisolone or other equivalent steroids may be used at the same dosing 1, 3

Condition-Specific Dosing

  • The American College of Rheumatology recommends first-line treatment for Multisystem Inflammatory Syndrome in Children (MIS-C) with Methylprednisolone 1-2 mg/kg/day IV, often combined with IVIG 2 g/kg 1, 2
  • The American College of Rheumatology recommends intensification treatment for MIS-C with Methylprednisolone 10-30 mg/kg/day IV for refractory disease (persistent fevers and/or ongoing significant end-organ involvement) 1, 2
  • Low-to-moderate dose glucocorticoids (1-2 mg/kg/day) may be considered as first-line therapy in MIS-C patients with concerning features such as ill appearance, highly elevated symptoms, or unexplained tachycardia 2

Important Monitoring and Precautions

  • Regular assessment for steroid-related adverse effects is essential, especially with prolonged therapy 4, 5
  • Consider baseline and annual bone mineral density testing for long-term therapy 4
  • Growth monitoring is particularly important in children on extended courses 5
  • Consider steroid-sparing agents if side effects become problematic 4, 5

Common Pitfalls and Caveats

  • For patients with cardiac dysfunction receiving high-dose therapy, monitor cardiac function and fluid status carefully 1, 2

Pediatric Methylprednisolone Dosing Guidelines

Standard Dosing and Monitoring

  • The American Academy of Allergy, Asthma, and Immunology recommends an initial dose of 1-2 mg/kg/day IV divided every 6 hours for acute asthma exacerbations, with a maximum dose of 60-80 mg/day 6
  • For asthma exacerbations, treatment should continue until peak expiratory flow reaches 70% of predicted, typically 3-10 days total 6
  • Short courses of methylprednisolone (<7-10 days) do not require tapering, but longer courses should be tapered gradually, especially if not on concurrent inhaled corticosteroids 6
  • Clinical improvement in respiratory status, fever resolution, or disease-specific markers should be monitored, and for asthma, peak expiratory flow should be monitored until it reaches 70% predicted 6

Dosing Considerations

  • Methylprednisolone is typically dosed by weight (mg/kg), and for significantly overweight children, dosing should be based on ideal body weight to avoid excessive steroid exposure 6
  • When transitioning from IV methylprednisolone to oral prednisone/prednisolone, an equivalency of methylprednisolone 1 mg = prednisone 1.25 mg (approximately 4:5 ratio) should be used, and oral dosing should be 1-2 mg/kg/day prednisone (maximum 60 mg/day) as a single morning dose 6
