Antiplatelet Loading and Maintenance in Acute Coronary Syndrome
Aspirin Loading and Maintenance
- Give an immediate loading dose of 162–325 mg non‑enteric‑coated, chewable aspirin to every patient with ACS as soon as possible after presentation, regardless of prior aspirin use. This rapid‑onset strategy is recommended by the American College of Cardiology. 1
- Chew the aspirin when feasible to achieve the fastest antiplatelet effect. Supported by ACC guidance. 1
- Avoid enteric‑coated aspirin formulations for the initial dose because they delay and diminish absorption. 2
- Maintain aspirin at 75–100 mg daily (rather than higher doses) to lower bleeding risk without sacrificing efficacy. 1
- Intravenous aspirin may be used when available as an alternative route in the acute setting. 1
- If oral administration is impossible, a rectal 325 mg aspirin dose is an acceptable alternative. (Praxis Medical Insights) 3
P2Y12 Inhibitor Loading Doses
Non‑ST‑Elevation ACS (NSTE‑ACS) – Early Invasive Strategy (angiography ≤ 24 h)
- Clopidogrel 300–600 mg loading (600 mg preferred for faster platelet inhibition), then 75 mg daily. Recommended by the ACC. 2
- Ticagrelor 180 mg loading, then 90 mg twice daily; preferred over clopidogrel for superior efficacy. ACC guidance. 1
- Prasugrel 60 mg loading at the time of PCI (after coronary anatomy is defined), then 10 mg daily in patients ≥ 60 kg and < 75 years. ACC recommendation. 1
Non‑ST‑Elevation ACS – Delayed Invasive or Ischemia‑Guided Strategy (angiography > 24 h or conservative)
- Administer clopidogrel or ticagrelor upstream (before angiography) using the same loading doses as above. This upstream strategy is endorsed by the ACC. 2
- Upstream loading reduces major adverse cardiovascular events (MACE) when angiography is expected after 24 h. Evidence from ACC data. 1
ST‑Elevation Myocardial Infarction (STEMI) – Primary PCI
- Prasugrel 60 mg or ticagrelor 180 mg should be given before PCI to lower MACE and stent‑thrombosis rates. ACC guideline. 1
- If prasugrel or ticagrelor are unavailable, contraindicated, or not tolerated, clopidogrel 300–600 mg loading is recommended. ACC guidance. 1
STEMI – Fibrinolytic Therapy
- Clopidogrel 300 mg loading for patients ≤ 75 years (no loading for > 75 years, give 75 mg maintenance). Both doses are to be given concurrently with fibrinolytic therapy. ACC recommendation. 1
Special Dosing Considerations
Prasugrel‑Specific Restrictions
- Do not give prasugrel before coronary anatomy is defined (i.e., before PCI) in NSTE‑ACS. ACC safety recommendation. 2
- Reduce maintenance dose to 5 mg daily in patients weighing < 60 kg. ACC dosing adjustment. 1
Ticagrelor‑Specific Considerations
- Aspirin maintenance must be ≤ 100 mg daily (81 mg preferred) when combined with ticagrelor; higher aspirin doses blunt ticagrelor’s antiplatelet effect. ACC guidance. 1
Management When Aspirin Is Not Tolerated
- Use a P2Y12 inhibitor alone (clopidogrel, prasugrel, or ticagrelor) with appropriate loading and maintenance dosing in patients with aspirin hypersensitivity or major gastrointestinal intolerance. Class I recommendation from the ACC. 2
Common Pitfalls to Avoid
- Do not use high‑dose aspirin maintenance (≥ 160 mg) – it increases bleeding without improving outcomes. ACC warning. 2
- Do not delay aspirin administration; it must be given immediately upon ACS recognition. ACC recommendation. 4
- Do not give prasugrel before coronary anatomy is known in NSTE‑ACS patients. ACC safety alert. 5
- Do not continue high‑dose aspirin with ticagrelor; keep aspirin ≤ 100 mg daily. ACC safety guidance. 1
- Do not start with enteric‑coated aspirin in the acute setting because of slow absorption. ACC recommendation. 2
