Antidepressants and Anti-Anxiety Medications with Least Renal Effects

First-Line Recommendations

  • The American Heart Association recommends sertraline as the preferred antidepressant for patients with impaired renal function due to its minimal direct nephrotoxicity and no requirement for renal dose adjustment 1, 2, 3
  • Sertraline has a lower risk of QTc prolongation compared to citalopram or escitalopram, making it a safer choice for patients with kidney disease 1, 2, 4
  • Mirtazapine is a safe alternative with additional benefits, including a demonstrated safety profile in cardiovascular disease and appetite stimulation, which may benefit malnourished CKD patients 1, 2, 4

Medications to Avoid in Renal Impairment

  • The American Heart Association recommends against the use of gabapentin and pregabalin in patients with compromised renal function due to the risk of fluid retention, weight gain, and toxicity 1, 2, 4, 5
  • Tricyclic antidepressants and monoamine oxidase inhibitors should be avoided entirely due to significant cardiovascular side effects, including hypotension and arrhythmias 1, 2, 4

Benzodiazepines for Anxiety

  • Diazepam and midazolam are safe choices for anxiety in renal patients, with no dose adjustment required due to liver metabolism 6
  • However, use with caution due to fall risk, particularly when combined with antihypertensives or diuretics 1, 4
