Statin Intolerance Management with Ezetimibe Combination Therapy

Introduction to Statin Alternatives

  • The European Society of Cardiology recommends switching to a different statin, such as fluvastatin, pravastatin, or low-dose atorvastatin, in combination with ezetimibe, rather than continuing rosuvastatin or using ezetimibe alone, for patients who experience myalgias 1, 2

Statin Re-Challenge Strategy

  • The American College of Cardiology suggests attempting at least three different statins before declaring true statin intolerance, with first-line alternatives including fluvastatin, pravastatin, and low-dose atorvastatin, in combination with ezetimibe 1, 2
  • The use of pitavastatin in combination with ezetimibe is particularly beneficial for patients with diabetes or metabolic syndrome, as it reduces the risk of new-onset diabetes 3

Combination Therapy Benefits

  • The American Heart Association recommends combination therapy with a statin and ezetimibe over monotherapy, as it achieves greater than 50% LDL-C reduction and produces fewer adverse events 4
  • Combination therapy with a low-dose statin and ezetimibe 10 mg produces greater LDL-C lowering than doubling the statin dose alone, with fewer adverse events 4
  • 94% of patients reach LDL-C goals less than 100 mg/dL with statin/ezetimibe combination therapy, compared to 79% with statin alone 4

Alternative Treatment Options

  • For patients who experience myalgias with multiple statins, consideration of bempedoic acid added to ezetimibe or PCSK9 inhibitors (alirocumab, evolocumab) or inclisiran added to maximally tolerated statin dose plus ezetimibe is recommended 2, 3
