Benzodiazepine Dose Equivalency

Standard Conversion Ratio

  • The American Academy of Pediatrics supports a 2:1 conversion ratio between lorazepam and clonazepam when converting between these benzodiazepines 1, 2, 3, 4
  • Clonazepam 1 mg is equivalent to lorazepam 2 mg, and therefore clonazepam 0.25 mg is equivalent to lorazepam 0.5 mg 5

Clinical Context for Conversion

  • The conversion ratio applies most reliably to anxiolysis and sedation purposes, as well as acute agitation management 2, 3, 5

Pharmacokinetic Differences

  • Lorazepam has a faster onset of action compared to clonazepam, with a peak effect at 3-5 minutes IV 5
  • Lorazepam is directly conjugated and eliminated, undergoing different metabolic pathways than clonazepam 6

Practical Dosing Considerations

  • For status epilepticus, weight-based dosing of lorazepam (0.05-0.10 mg/kg, maximum 4 mg) is preferred over fixed conversions 7
  • Patient factors such as age, liver function, and concomitant medications affect metabolism of both drugs 6

Safety Considerations

  • Both medications can cause respiratory depression, especially when combined with other sedatives or opioids, and require monitoring of oxygen saturation and preparation to provide respiratory support 5, 6, 7

Benzodiazepine Dose Conversion Considerations

Pharmacokinetic Differences

  • Clonazepam has a dramatically longer half-life (30-40 hours) compared to lorazepam, affecting dosing frequency and accumulation risk, and 90% bioavailability after oral administration, according to the American Academy of Sleep Medicine 8
  • Women may require higher doses than men to achieve the same clinical effect, as noted by the American Academy of Sleep Medicine 8

Monitoring for Side Effects

  • Morning sedation, early morning motor incoordination, confusion and memory dysfunction, risk of falls, and potential worsening of sleep apnea are common adverse effects with clonazepam, as reported by the American Academy of Sleep Medicine 8
  • Use clonazepam with particular caution in patients with neurodegenerative disorders, obstructive sleep apnea, or underlying liver disease, as advised by the American Academy of Sleep Medicine 8

Respiratory Depression Risk

  • Both medications can cause respiratory depression, especially when combined with opioids or other CNS depressants, requiring monitoring of oxygen saturation and preparation to provide respiratory support if needed, according to the Society of Critical Care Medicine 9
