Nasogastric Tube Placement in Anorexia Nervosa
Clinical Indications and Implementation
- The American College of Gastroenterology recommends using fine bore 5-8 French gauge NG tubes to minimize nasal and esophageal irritation in patients with anorexia nervosa 1, 2
- NG tubes are appropriate for feeding durations up to 4-6 weeks in patients with anorexia nervosa, as recommended by the American Gastroenterological Association 1, 3
- For feeding needs exceeding 4-6 weeks, consider percutaneous gastrostomy tubes, as suggested by the American Society for Parenteral and Enteral Nutrition 1, 3
- Verify tube position radiographically before initiating feeding to prevent complications, as recommended by the Society of Critical Care Medicine 3
- Monitor fluid, electrolytes, and glucose closely in the first few days of feeding to prevent refeeding syndrome, as recommended by the American Society for Parenteral and Enteral Nutrition 3, 4
- Start feeding within 24-48 hours after tube placement confirmation, and consider starting at 50-70% of target calories in severely malnourished patients, as recommended by the American Gastroenterological Association 3
- Secure the tube properly to prevent dislodgement, which occurs in 40-80% of cases without adequate securement, as reported by the American College of Gastroenterology 3, 2
- Consider nasal bridles for high-risk patients to reduce dislodgement rates, as suggested by the Society of Critical Care Medicine 3
- Never rely solely on auscultation to verify tube position, as it has only 79% sensitivity and 61% specificity, as reported by the American Thoracic Society 3
- Always obtain radiographic confirmation before initiating feeding to prevent complications, as recommended by the Radiological Society of North America 3
- Assess refeeding risk in all malnourished patients, and monitor electrolytes closely during the first 3-5 days of feeding, as recommended by the American Society for Parenteral and Enteral Nutrition 3, 4
