Management of Adenomyosis and Associated Conditions

Diagnostic Priorities

  • Endometrial biopsy is mandatory in a 41-year-old with amenorrhea and secretory endometrium to exclude endometrial pathology, particularly given the increased risk of endometrial cancer in diabetic patients 1, 2
  • The finding of secretory endometrium in the setting of amenorrhea is paradoxical and warrants tissue diagnosis to exclude hyperplasia or malignancy 3

First-Line Medical Management

  • The American College of Obstetricians and Gynecologists recommends the levonorgestrel-releasing intrauterine device (LNG-IUD) as first-line therapy for adenomyosis, reducing menstrual blood loss by 71-95% with efficacy comparable to endometrial ablation 2, 4
  • The LNG-IUD acts primarily at the endometrial level with minimal systemic progesterone absorption, making it suitable for diabetic patients 2

Alternative Hormonal Options

  • Combined oral contraceptives reduce painful and heavy menstrual bleeding, though less effective than LNG-IUD 2, 4
  • Cyclic oral progestin reduces bleeding by 87%, often resulting in only light bleeding 2
  • GnRH antagonists are highly effective for heavy menstrual bleeding even with concomitant adenomyosis 2, 4, 5

Diabetes-Specific Considerations

  • The American Diabetes Association recommends preconception counseling for diabetic patients, including discussion of risks of congenital anomalies and need for optimal glycemic control before conception 6
  • Effective contraception should be maintained until glycemia is well-controlled 6

Interventional Options

  • Uterine artery embolization (UAE) provides short-term improvement in 94% and long-term improvement in 85% of patients with symptom control up to 7 years 2, 4, 5
  • Hysterectomy should be offered when medical and interventional therapies fail, providing definitive resolution with patient satisfaction rates up to 90% 2

Progestin-Only Contraceptive for Adenomyosis

Contraception and Symptom Management

  • The American College of Cardiology is not relevant here, however, it is noted that all progestin-only options provide contraception, which is advantageous for women not desiring pregnancy while managing adenomyosis symptoms 7

Introduction to Progestin-Only Options

  • No relevant facts were found with a citation id, however the first fact does have one,
  • All these progestin-only options, including the one mentioned, provide contraception, which is advantageous for women not desiring pregnancy while managing adenomyosis symptoms 7
