Eritromelalgia Management

Diagnosis and Initial Assessment

  • The American College of Physicians suggests that eritromelalgia is highly suggestive, especially given the pattern of bilateral edema with erythema, pain that improves with local cold, and the fluctuating nature of symptoms 1, 2
  • Bilateral hand edema with erythema that appears episodically is a characteristic of eritromelalgia 1
  • Improvement with local cold is a pathognomonic feature of eritromelalgia 1, 2
  • Moderate pain (ENA 5/10) is associated with episodes 1

Treatment Approach

  • The American Academy of Family Physicians recommends avoiding triggers such as excessive heat, strenuous exercise, and prolonged dependence on hands 1, 2
  • Limiting cooling to 10 minutes maximum, 4 times a day, and warning about tissue damage from prolonged immersion in cold water 2
  • Elevating the extremities during episodes 1
  • Topical therapy, such as lidocaine 4% or 5% patches, is a preferred first-line treatment 2
  • Aspirin 325 mg orally daily for at least 1 month is effective, especially if an underlying myeloproliferative disorder is identified 4, 2

Management of Refractory Cases

  • Referring to a comprehensive pain rehabilitation center for multidisciplinary management is recommended for cases that do not respond to first and second-line therapies 1, 2
  • Considering lidocaine intravenously (with caution due to neurological effects) or ketamine infusion (with limited evidence and controversy due to adverse effects) 2

Special Considerations

  • The American College of Physicians advises against discontinuing treatments prematurely, as topical treatments require a 4-week trial and systemic treatments require a 3-4 month trial 2
  • Excessive cooling can cause tissue damage and ulcerations, and symptoms can worsen paradoxically upon discontinuation of cold therapy 2
  • Limiting cooling strictly to 10 minutes, maximum 4 times a day, is recommended 2
