Treatment of Fungal Intertrigo

Initial Management Approach

  • The American College of Physicians recommends initiating topical azole therapy (clotrimazole, miconazole, or ketoconazole) applied twice daily for 2-4 weeks, reserving oral fluconazole 150-200 mg daily for 14 days for resistant or extensive cases, for patients with confirmed candidal intertrigo (positive KOH with fungal spores) 1

Pharmacologic Treatment Algorithm

  • The Infectious Diseases Society of America suggests applying topical azoles (clotrimazole, miconazole, ketoconazole) or nystatin twice daily to affected areas, as these are effective for primary candidal skin infections and intertrigo in most cases 1
  • The Clinical Infectious Diseases guideline recommends treating for a minimum of 14 days for symptomatic candidal infections 1
  • The Praxis Medical Insights guideline suggests reserving oral fluconazole for treatment-resistant cases, extensive involvement, or when topical therapy fails, with a recommended dosing of fluconazole 150-200 mg once daily for 14 days 2

Treatment Duration and Monitoring

  • The Clinical Infectious Diseases guideline recommends treating until complete resolution of signs and symptoms, and continuing therapy for at least 2-4 weeks even after clinical resolution to prevent recurrence 1
  • The Praxis Medical Insights guideline suggests scheduling follow-up within 1-2 weeks after completing therapy to ensure resolution 2
