Meropenem Reconstitution with Lidocaine

Rationale for Using Lidocaine for Reconstitution

  • Lidocaine helps minimize pain during administration while maintaining the efficacy of the medication being reconstituted 1
  • Using 1% lidocaine solution provides adequate anesthetic effect while minimizing potential systemic effects 1

Safety Considerations

  • The maximum safe dose of lidocaine with epinephrine is 7 mg/kg in adults and 3.0-4.5 mg/kg in children 2
  • Without epinephrine, maximum doses are 4.5 mg/kg for adults and 1.5-2.0 mg/kg for children 2
  • Monitor for signs of local anesthetic systemic toxicity (LAST), including circumoral numbness, facial tingling, slurred speech, and metallic taste 2
  • Avoid using in patients with known lidocaine allergy (rare, but possible) 2

Alternative Options

  • Bacteriostatic normal saline (0.9% benzyl alcohol in normal saline) can be used as an alternative 2
  • For patients with true lidocaine allergy, consider using ester-type local anesthetics or 1% diphenhydramine 2

Administration Techniques to Minimize Pain

  • Use slow infiltration rate when administering 1
  • Consider warming the solution to approximately 40°C 1
  • Buffering the lidocaine with sodium bicarbonate in a 1:9 or 1:10 ratio can further reduce pain, though this is not necessary for the initial reconstitution 1
