Ciprofloxacin Dosing in Renal Impairment
Introduction to Renal Dosing
- The American College of Clinical Pharmacology recommends calculating creatinine clearance using the Cockcroft-Gault formula to avoid assuming normal dosing is safe based solely on "normal" serum creatinine in elderly or sarcopenic patients, as muscle mass decline can mask severe renal impairment 1
Drug Interactions and Administration
- The Infectious Diseases Society of America advises administering ciprofloxacin at least 2 hours before or 6 hours after products containing divalent or trivalent cations (magnesium/aluminum antacids, calcium, iron, zinc supplements) as these dramatically reduce absorption 2
- The National Kidney Foundation recommends never administering ciprofloxacin before hemodialysis as this results in premature drug removal and subtherapeutic levels 2
Monitoring and Special Considerations
- The Clinical Infectious Diseases society suggests monitoring for CNS adverse effects (dizziness, headache, insomnia) which may be more common with accumulation in renal impairment 2
- The European Renal Association recommends considering therapeutic drug monitoring in severe renal impairment to ensure adequate exposure without toxicity 3
