Cefuroxime Surgical Prophylaxis Dosing

Standard Dose Recommendation

  • The standard dose of cefuroxime for most surgical procedures is 1.5 g IV as a single dose within 30-60 minutes before surgical incision, with re-dosing of 0.75 g every 2 hours intraoperatively if the procedure is prolonged, as recommended by the Anaesthesia guideline society 1

Procedure-Specific Dosing

  • For cardiac surgery, the initial dose is 1.5 g IV plus 0.75 g in the CPB priming solution, with intraoperative re-injection of 0.75 g every 2 hours, as recommended by the Anaesthesia guideline society 1
  • For vascular surgery, the initial dose is 1.5 g IV slow infusion, with re-injection of 0.75 g if procedure duration exceeds 2 hours, as recommended by the Anaesthesia guideline society 1
  • For cataract surgery, the dose is 1 mg in 0.1 mL at the end of the procedure, representing a unique route and dose specific to ophthalmology, as recommended by the Anaesthesia guideline society 1

Critical Timing Considerations

  • The optimal administration window is 30-60 minutes before incision to ensure adequate tissue concentrations at the time of potential contamination, as recommended by the Anaesthesia and Praxis Medical Insights guideline societies 1, 2
  • The maximum acceptable timing is infusion must be completed no later than the beginning of the intervention, as recommended by the Anaesthesia guideline society 1
  • Giving prophylaxis more than 120 minutes before incision is unnecessary and potentially dangerous, as recommended by the Praxis Medical Insights guideline society 2

Intraoperative Re-dosing Guidelines

  • Re-dose when surgical duration exceeds 2 hours, with a re-dosing amount of 0.75 g IV, as recommended by the Anaesthesia and Praxis Medical Insights guideline societies 1, 2
  • For cardiac surgery, re-inject 0.75 g every 2 hours intraoperatively, as recommended by the Anaesthesia guideline society 1

Duration of Prophylaxis

  • The standard duration is a single preoperative dose, which is adequate for most procedures, as recommended by the Praxis Medical Insights guideline society 2
  • The maximum duration should not exceed 24 hours postoperatively, as recommended by the Praxis Medical Insights guideline society 2, 3
  • Postoperative doses beyond the operative period provide no additional benefit and increase antibiotic resistance risk, as recommended by the Praxis Medical Insights guideline society 2

Special Populations

  • For morbidly obese patients ≥120 kg, higher doses may be required to achieve adequate tissue concentrations, as recommended by the Praxis Medical Insights guideline society 2

Clinical Caveats and Pitfalls

  • Do not continue prophylaxis beyond 24 hours unless specifically indicated, as this increases resistance without benefit, as recommended by the Praxis Medical Insights guideline society 2
  • Do not use inadequate dosing in obese patients, as tissue penetration may be compromised, as recommended by the Praxis Medical Insights guideline society 2
  • Do not administer too early (>120 minutes before incision), as recommended by the Praxis Medical Insights guideline society 2
