Oral Steroid Treatment for Contact Dermatitis
Treatment Selection and Protocol
- For mild to moderate contact dermatitis, the American Academy of Dermatology recommends using topical steroids as first-line treatment, with low to mid-potency for sensitive areas and mid to high-potency for other body areas 1
- For extensive or severe contact dermatitis (>20% body surface area), oral prednisone is indicated, with a starting dose of 0.5-1 mg/kg/day and a duration of 2-3 weeks with gradual taper to prevent rebound dermatitis 2
- The British Association of Dermatologists suggests that oral steroids allow for precise dose adjustments based on clinical response and individualized tapering schedules to prevent rebound dermatitis 3, 4
Adjunctive Measures and Monitoring
- The National Eczema Association recommends applying emollients regularly to maintain skin hydration, identifying and eliminating potential irritants, and using soap substitutes for cleansing affected areas 1
- Consider antihistamines for pruritus control, as suggested by the Journal for ImmunoTherapy of Cancer 2
- Follow up within 1 week to assess response and monitor for signs of steroid-related adverse effects, as recommended by the Journal for ImmunoTherapy of Cancer and Praxis Medical Insights 1, 2
