Antibiotic Treatment for Providencia stuartii Infections
Antimicrobial Selection
- Ciprofloxacin (400mg IV every 12h or 750mg PO every 12h) may be used for susceptible strains, especially in urinary tract infections, as recommended by the Infectious Diseases Society of America 1, 2
- Piperacillin-tazobactam (3.375g every 6h or 4.5g every 8h IV) is an option for polymicrobial infections involving P. stuartii, according to the Clinical Infectious Diseases guidelines 1, 2
- Ciprofloxacin plus metronidazole may be used for infections near axilla or perineum, as suggested by the Clinical Infectious Diseases guidelines 2
Treatment Duration
- For most bacterial skin and soft tissue infections, 7-14 days of treatment is recommended, as stated by the Clinical Infectious Diseases guidelines 3
- For bacteremia, 2-3 weeks of therapy is typically recommended, according to the Clinical Infectious Diseases guidelines 1
- Initial IV therapy can be switched to oral antibiotics once clinical improvement is observed, as recommended by the Clinical Infectious Diseases guidelines 1
Special Considerations
- Avoid empiric use of carbapenems unless multidrug resistance is documented to prevent further resistance development, as advised by the World Journal of Emergency Surgery guidelines 5
- Bacteriophage therapy is being investigated as a potential treatment for biofilm-associated infections, as reported by The Lancet Microbe 4
- For persistent bacteremia, perform repeat imaging studies to identify undrained foci of infection, as recommended by the Clinical Infectious Diseases guidelines 1
