Magnesium Sulfate Administration and Serum Magnesium Levels

Clinical Applications and Dosing

  • The American Heart Association recommends 2 grams of magnesium sulfate infused intravenously as a first-line agent for treatment of torsades de pointes, regardless of baseline serum magnesium level 1
  • For severe asthma exacerbations, the standard adult dose is 2 grams administered over 20 minutes 2, 3

Monitoring and Safety Considerations

  • Common side effects of rapid magnesium administration include flushing, feeling of warmth, and light-headedness 2

Oral vs. Intravenous Administration

  • Oral magnesium oxide contains approximately 60% elemental magnesium, with typical daily doses of 12-24 mmol for hypomagnesemia treatment 4
  • Oral magnesium is best administered at night when intestinal transit is slower, allowing more time for absorption 4
  • In patients with short bowel syndrome or significant GI disease, oral absorption may be further compromised, requiring higher doses or parenteral administration 4

Clinical Pearls

  • Magnesium sulfate is effective regardless of baseline serum magnesium levels for certain conditions like torsades de pointes 1
