Iron Sucrose Administration Guidelines

Maximum Dosing Limits

  • The National Comprehensive Cancer Network recommends that individual doses of iron sucrose above 300 mg are not recommended, and the maximum weekly dose is 500 mg, as per Kidney International guidelines 1, 2, 3

Safety Considerations for Frequent Dosing

  • Active infection is an absolute contraindication to IV iron therapy, as recommended by the National Comprehensive Cancer Network and Kidney International guidelines 1, 2, 3
  • The Kidney International guidelines recommend monitoring vital signs during and after each infusion to detect hypotension, flushing, or other reactions 3
  • Resuscitation facilities should be available during administration due to potential anaphylaxis risk, as recommended by the Kidney International guidelines 3
  • Test doses (25 mg slow IV push) are strongly recommended for patients with prior sensitivities to IV iron preparations or multiple drug allergies, as recommended by the National Comprehensive Cancer Network and Kidney International guidelines 1, 2, 3

Common Pitfalls to Avoid

  • The National Comprehensive Cancer Network recommends not exceeding 200 mg per individual dose in routine practice, and not administering more than 500 mg per week, as per Kidney International guidelines 1, 2, 3
  • The National Comprehensive Cancer Network and Kidney International guidelines recommend not giving IV iron to patients with active infection 1, 2, 3
