Vitamin D Deficiency Diagnosis and Management

Common Clinical Manifestations

  • Vitamin D deficiency commonly presents with fatigue, weakness, bone pain, muscle aches, and increased risk of fractures, and should be suspected in patients with these symptoms, particularly those with risk factors for low vitamin D levels 1

High-Risk Populations

  • Individuals with limited sun exposure or who routinely wear clothing that prevents sun exposure are at high risk of vitamin D deficiency, according to the American College of Physicians 2
  • People with darker skin pigmentation, older adults, patients with malabsorptive conditions, obese individuals, homebound or institutionalized persons, and those living at high latitudes are also at high risk of vitamin D deficiency 2
  • Patients with malabsorptive conditions, inflammatory bowel disease, or history of gastric bypass are at high risk of vitamin D deficiency, as stated by the Praxis Medical Insights 1, 2

Laboratory Findings

  • Serum 25-hydroxyvitamin D level below 20 ng/mL is defined as vitamin D deficiency, as per the Praxis Medical Insights 1

Clinical Pitfalls to Avoid

  • Not recognizing that vitamin D requirements vary by individual based on risk factors, as stated by the Praxis Medical Insights 1
  • Not considering that total serum 25-(OH)D measurements may not accurately reflect vitamin D status in all populations, particularly African Americans, according to the American College of Physicians 2

Vitamin D Deficiency Diagnosis and Prevention

Diagnostic Considerations

  • Older age combined with winter season are predictive factors for vitamin D deficiency, according to the American Academy of Family Physicians, highlighting the importance of seasonal timing when evaluating symptoms 3

Population-Specific Risk Factors

  • The National Institute on Aging recommends considering vitamin D deficiency as a major cause of frailty syndrome in older adults, which contributes to sarcopenia and increases risk of falls, fractures, physical disabilities, and reduced quality of life, with a moderate level of evidence 3
