Oral Iron and Antacid Interaction
Mechanism of Interaction
- Aluminum-based phosphate binders reduce iron absorption, as they neutralize stomach acid, preventing the conversion of ferric iron to the more absorbable ferrous form 1, 2
Clinical Recommendations
- The American Journal of Kidney Diseases recommends separating administration times, taking oral iron at least 2 hours before or 1 hour after antacid use, to optimize iron therapy 1, 2
- Taking iron on an empty stomach when possible is recommended for maximum absorption, as suggested by the American Journal of Kidney Diseases and Clinical Gastroenterology and Hepatology 1, 2, 3
- Consider vitamin C supplementation (80-500 mg) with iron to enhance absorption and potentially overcome some antacid effects, as recommended by Clinical Gastroenterology and Hepatology 3, 4
