Management of Uncontrolled Hypertension on Amlodipine 5 mg

Treatment Intensification for Stage 2 Hypertension

  • Patients with stage 2 hypertension (systolic ≥ 140 mm Hg) should receive immediate pharmacologic intensification, independent of overall cardiovascular risk. 1, 3
  • The universal blood‑pressure goal for adults is < 130/80 mm Hg; achieving a systolic range of 120–129 mm Hg when tolerated further reduces cardiovascular events. 1, 2
  • Confirmation of true hypertension should be obtained with home blood‑pressure monitoring or repeat office measurements to exclude white‑coat effect. 1

Preferred Combination Strategies

  • Add a second antihypertensive from a different class—either a renin‑angiotensin‑system (RAS) blocker (ACE inhibitor or ARB) or a thiazide/thiazide‑like diuretic—as combination therapy, ideally as a single‑pill fixed‑dose formulation to improve adherence and accelerate control. 1, 2, 3
  • For patients inadequately controlled on calcium‑channel‑blocker monotherapy, a fixed‑dose combination of amlodipine 5 mg plus an ACE inhibitor or ARB is the recommended first‑line approach. 2, 3
  • When a RAS blocker is contraindicated or not tolerated, adding a thiazide or thiazide‑like diuretic (e.g., chlorthalidone or indapamide) to amlodipine provides an effective alternative. 2, 3
  • Fixed‑dose combinations achieve faster time‑to‑target blood pressure compared with simply uptitrating amlodipine to 10 mg. 1

Specific Preferred Fixed‑Dose Options

Amlodipine (5 mg) Partner Drug (Fixed‑Dose)
+ ARB (e.g., olmesartan 20 mg, valsartan 160 mg, candesartan) Single‑pill ARB combo
+ ACE inhibitor (e.g., perindopril 4 mg) Single‑pill ACEI combo
+ Thiazide‑like diuretic (if RAS blocker contraindicated) Single‑pill diuretic combo

These combinations are endorsed by major cardiovascular societies. 2, 3

Implementation and Monitoring

  • Initiate the combination therapy immediately after confirming uncontrolled blood pressure; do not postpone for lifestyle changes alone. 1, 3
  • Re‑evaluate blood pressure after 4 weeks; if still above target, intensify to a three‑drug regimen (amlodipine + RAS blocker + thiazide/thiazide‑like diuretic). 2, 3
  • Home blood‑pressure monitoring should be incorporated into routine follow‑up to avoid reliance solely on office readings. 1

Contraindications and Common Pitfalls

  • Combining two RAS blockers (ACE inhibitor + ARB) is explicitly discouraged. 2, 3
  • Using separate pills when an equivalent fixed‑dose combination exists reduces adherence and should be avoided. 1, 2

Blood‑Pressure Targets and Expected Benefits

  • Aim for a systolic blood pressure of 120–129 mm Hg (if well tolerated) with an absolute target of < 130/80 mm Hg. 1, 2
  • Achieving these targets is associated with reduced cardiovascular morbidity and mortality across all patient populations. 1
