Pharmacodynamic Interaction Between Bethanechol and Salbutamol

Mechanism of Action

  • Salbutamol is a β2‑adrenergic agonist that induces bronchodilation by relaxing bronchial smooth muscle, whereas bethanechol is a muscarinic cholinergic agonist that promotes bronchoconstriction; their opposing actions create a direct pharmacodynamic antagonism. 1

Respiratory Risks

  • In patients with obstructive airway diseases (e.g., asthma or chronic obstructive pulmonary disease), the bronchoconstrictive effect of bethanechol can diminish or negate the bronchodilator effect of salbutamol, increasing the likelihood of acute bronchospasm. 3
  • The combination may trigger bronchospasm in susceptible individuals, manifested by wheezing, elevated respiratory rate, and difficulty speaking full sentences. 4

Cardiovascular Risks

  • Salbutamol administration is associated with a mean increase of approximately 9 beats per minute in heart rate, reflecting a tachycardic effect. 1
  • When used together, the tachycardic and vasomotor responses of both drugs may be additive, raising the risk of adverse cardiac events. 1

Contraindication Assessment

  • Concurrent use of bethanechol and salbutamol should be avoided in patients with:

Monitoring Recommendations

  • If co‑administration cannot be avoided, closely monitor respiratory status for signs of bronchospasm (wheezing, respiratory rate > 25 /min, speech difficulty). 4
  • Assess cardiovascular parameters, including heart rate and blood pressure, to detect additive tachycardia.
  • Evaluate salbutamol efficacy by measuring peak expiratory flow or other spirometric indices to identify reduced bronchodilator response. 3

Alternative Strategies

  • Consider alternative agents to bethanechol that lack bronchoconstrictive properties when patients require β2‑agonist therapy, thereby minimizing the risk of antagonistic interaction. 1
  • Prior to initiating bethanechol in any patient receiving salbutamol, obtain a thorough respiratory history to identify underlying asthma, COPD, or reactive airway disease. 1
