Pseudohyperkalemia Diagnosis and Management

Common Causes and Diagnostic Approach

  • Poor phlebotomy techniques, including excessive fist clenching during blood collection and hemolysis during sample collection or processing, can lead to pseudohyperkalemia in patients with normal effective plasma potassium concentration, according to the Mayo Clinic Proceedings 1, 2, 3
  • Sample processing issues, such as delayed specimen processing, can contribute to pseudohyperkalemia in patients with elevated potassium levels, as reported by the Mayo Clinic Proceedings 1, 3
  • When hyperkalemia is detected, checking for clinical symptoms, such as muscle weakness and paresthesias, and obtaining an ECG to look for hyperkalemia changes, can help diagnose pseudohyperkalemia, as recommended by the Mayo Clinic Proceedings 1, 4
  • The European Society of Cardiology warns against failing to rule out pseudohyperkalemia before initiating treatment for hyperkalemia, highlighting the importance of proper diagnosis and management 5

Management and Special Considerations

  • Proper phlebotomy techniques, such as minimal tourniquet time and avoiding excessive fist clenching, and prompt sample processing can help prevent pseudohyperkalemia in patients with known pseudohyperkalemia, as suggested by the Mayo Clinic Proceedings 1, 3
