Olanzapine Administration Timing Considerations

Dosing and Administration

  • The American Psychiatric Association recommends that olanzapine can be taken at any consistent time of day, allowing for flexible dosing schedules, with a long half-life supporting once-daily dosing 1
  • Morning administration may reduce interference with daytime functioning if sedation occurs, and allows sedative effects to occur during evening/nighttime hours when they may be less problematic 1
  • Incorporating olanzapine into morning routines may improve medication adherence 2

Dosing Guidelines

  • The National Institute for Health and Care Excellence recommends initial target doses for early psychosis of 7.5-10 mg/day, with a therapeutic range of 5-20 mg/day for schizophrenia and bipolar disorder 3, 2
  • Doses should be individualized based on efficacy and tolerability, with assessment after at least 4 weeks at therapeutic doses 2

Special Populations

  • The American Geriatrics Society recommends that elderly patients require special caution due to increased mortality in dementia-related psychosis 1
