Dexmedetomidine Dosing in Patients with Renal Insufficiency on Hemodialysis

Introduction to Dexmedetomidine Administration

  • The American Society of Health-System Pharmacists recommends administering 1 μg/kg IV during 10 minutes in hemodynamically stable patients, with a maintenance infusion of 0.2-0.7 μg/kg/hour 1, 2
  • In patients with hemodynamic instability, the dose of dexmedetomidine should be omitted to avoid transient hypertension followed by hypotension within 5-10 minutes 2, 3

Dosage and Administration

  • The maintenance infusion can be titrated up to 1.5 μg/kg/hour based on tolerance and desired level of sedation 1, 2
  • For a 70 kg patient, the loading dose is 70 mcg = 17.5 mL infused over 10 minutes, and the initial maintenance infusion is 35 mcg/hour = 8.75 mL/hour 2

Monitoring and Contraindications

  • The Society of Critical Care Medicine recommends continuous hemodynamic monitoring during dexmedetomidine administration, with blood pressure and heart rate checks every 2-3 minutes during the loading dose 2, 4
  • Contraindications include sinus node disease, second- or third-degree AV block, and severe hemodynamic instability 4

Special Considerations

  • The American College of Clinical Pharmacy notes that dexmedetomidine may cause loss of oropharyngeal muscle tone, leading to airway obstruction in non-intubated patients, requiring continuous respiratory monitoring 2, 4
  • In non-intubated patients, pulse oximetry is mandatory 2
