Co‑Administration of Vyvanse (Lisdexamfetamine) and Hydroxyzine: Safety, Dosing, and Monitoring

Safety Considerations

  • Hydroxyzine should not be combined with other central‑ nervous system depressants (e.g., tricyclic antidepressants, benzodiazepines, opioids) because of additive sedation and performance impairment. Society for Immunotherapy of Cancer guideline. 2
  • The warning against concurrent CNS depressants does not apply to stimulants such as Vyvanse, so the two agents can be used together when timing is separated. Society for Immunotherapy of Cancer guideline. 2
  • Hydroxyzine can prolong the QT interval, which is a concern only when it is taken with other QT‑prolonging drugs (e.g., tricyclic antidepressants). Society for Immunotherapy of Cancer guideline. 2
  • Hydroxyzine induces sedation and performance impairment even when patients do not feel drowsy; drivers taking hydroxyzine are about 1.5 × more likely to be responsible for fatal motor‑vehicle accidents. Praxis Medical Insights guideline. 1
  • Avoid multiple daily doses of hydroxyzine because this markedly raises the risk of daytime drowsiness, cognitive slowing, and interference with the stimulant effect of Vyvanse. Praxis Medical Insights guideline. 1

Dosing Recommendations and Timing

  • Vyvanse should be taken in the morning; start at 20–30 mg once daily, titrating up to 70 mg as needed, providing 12–14 hours of ADHD symptom control. American College of Obstetricians and Gynecologists recommendation. 3
  • Hydroxyzine should be reserved for bedtime use only, with a dose range of 10–50 mg (adjusted for age and organ function) when needed for anxiety, pruritus, or urticaria. Praxis Medical Insights guideline. 1, 4
  • This temporal separation (Vyvanse morning, hydroxyzine at night) minimizes any pharmacodynamic interaction between the stimulant and the sedative effects. Praxis Medical Insights guideline. 1

Special Populations

Population Hydroxyzine Adjustment Comment
Moderate renal impairment (CrCl 10–20 mL/min) Reduce dose by 50 % Praxis Medical Insights guideline. [1]
Severe renal impairment (CrCl < 10 mL/min) Avoid hydroxyzine Praxis Medical Insights guideline. [1]
Severe hepatic disease Avoid hydroxyzine Praxis Medical Insights guideline. [1]
Elderly Start at low end of range (≈10 mg at bedtime); monitor for falls, anticholinergic effects, and cognitive decline Praxis Medical Insights guideline. [1]
Early pregnancy (first trimester) Contraindicated Praxis Medical Insights guideline. [1]

Monitoring, Counseling, and Pitfalls

  • Counsel patients that sedation can occur without subjective drowsiness and that they should avoid driving or operating machinery after taking hydroxyzine, even if they feel alert. Praxis Medical Insights guideline. 1
  • Monitor for residual daytime sedation that could blunt Vyvanse’s therapeutic effect; adjust hydroxyzine timing or dose if needed. Praxis Medical Insights guideline. 1
  • Do not use hydroxyzine during daytime hours while Vyvanse is active, as this defeats stimulant therapy and raises accident risk. Praxis Medical Insights guideline. 1
  • Avoid combining hydroxyzine with other anticholinergic agents, especially in older adults with prostatic hypertrophy, elevated intra‑ocular pressure, or cognitive impairment. Praxis Medical Insights guideline. 1
