Managing Acid Reflux in Patients Taking Propranolol

Initial Management Approach

  • The American Gastroenterological Association recommends managing acid reflux in patients on propranolol using the same evidence-based approach as for any GERD patient, starting with lifestyle modifications and a 4-8 week trial of single-dose PPI therapy taken 30-60 minutes before meals 1, 2
  • Dietary modifications, such as limiting fat intake to <45g per 24 hours, avoiding coffee, tea, soda, chocolate, mints, citrus products, and alcohol, are recommended for patients with acid reflux 5

Pharmacologic Therapy

  • The American College of Gastroenterology recommends starting with single-dose PPI once daily, taken 30-60 minutes before a meal for 4-8 weeks, as the primary treatment for acid reflux 1, 2
  • Any commercially available PPI is appropriate initially, and reassessing symptoms after 4-8 weeks is recommended 1, 2

If Inadequate Response After Initial Trial

  • Increasing to twice-daily dosing or switching to a more potent PPI, such as rabeprazole or esomeprazole, may be necessary for patients who do not respond to initial PPI therapy 1, 2
  • Alginate-containing antacids, H2-receptor antagonists, baclofen, and prokinetics may be used as adjunctive pharmacotherapy for patients with persistent symptoms 1, 6

Long-Term Management

  • Tapering to the lowest effective dose or switching to H2RAs/antacids is recommended for patients whose symptoms are controlled 1, 7
  • Exceptions requiring long-term PPI include Los Angeles B or greater esophagitis, Barrett's esophagus, or peptic stricture 1, 7

Important Clinical Considerations

  • Beta-blockers like propranolol do not interfere with standard GERD therapies, and there is no need to modify or discontinue propranolol for GERD management unless it is one of the rare medications that worsens reflux 1, 5, 7
  • Emphasizing PPI safety to patients is important, as they have safety profiles comparable to placebo for short-term use 3
