Prednisone Burst Dosing Guidelines
Introduction to Prednisone Burst Dosing
- For adolescents ≥12 years of age, the American Academy of Allergy, Asthma, and Immunology recommends a prednisone dose of 40-60 mg/day for 3-10 days, with no need to taper for courses under 10 days 1, 2
Specific Dosing Recommendations
- The standard burst regimen for adolescents ≥12 years of age is 40-60 mg/day, which can be given as a single daily dose or divided into 2 doses, with a duration of 3-10 days 1, 2, 3
- For weight-based dosing, the recommended dose is 1-2 mg/kg/day with a maximum of 60 mg/day, and dosing should be based on ideal body weight if the patient is significantly overweight 1, 2, 4
Clinical Context and Indications
- The National Heart, Lung, and Blood Institute guidelines recommend 40-60 mg/day for outpatient bursts in patients ≥12 years, which is effective for establishing control when initiating therapy or during periods of gradual deterioration 1, 2, 7
- No advantage exists for higher doses in asthma exacerbations, and a 2-day course of dexamethasone may be considered as an alternative 2, 3
Important Clinical Considerations
- Common adverse effects of prednisone include reversible abnormalities in glucose metabolism, increased appetite, fluid retention, weight gain, mood alteration, hypertension, insomnia, and facial flushing, which are generally well-tolerated and resolve after discontinuation 1, 7
- Courses <1 week do not require tapering, and courses up to 10 days probably do not need tapering, especially if the patient is on inhaled corticosteroids 2, 3
Monitoring and Follow-Up
- Ensure the patient has appropriate follow-up to assess response, and consider coexisting conditions that could be worsened by systemic corticosteroids (e.g., diabetes, hypertension, peptic ulcer) 1
- No advantage exists for IV administration over oral therapy if gastrointestinal absorption is intact, and inhaled corticosteroids can be started at any point during treatment of an exacerbation 2, 3
