Duration of Apixaban Treatment for Deep Vein Thrombosis (DVT)
Treatment Duration Based on DVT Classification
- The American College of Chest Physicians recommends 3 months of apixaban treatment for DVT provoked by surgery or a non-surgical transient risk factor, such as trauma or pregnancy, in patients with low to moderate bleeding risk 1, 2, 3
- For unprovoked DVT, the American College of Chest Physicians recommends treatment with anticoagulation for at least 3 months, followed by evaluation for extended therapy based on bleeding risk 4, 3
- The American College of Cardiology suggests extended anticoagulant therapy for patients with a first unprovoked proximal DVT and low or moderate bleeding risk 3
Apixaban Dosing Regimen
- The American College of Cardiology recommends an initial apixaban treatment of 10 mg orally twice daily for the first 7 days, followed by a maintenance treatment of 5 mg orally twice daily 5
- For extended therapy beyond 6 months, a reduced dose of 2.5 mg twice daily can be considered, according to the American College of Cardiology 6, 5
Special Considerations
- The American College of Chest Physicians recommends reassessing the continuing use of extended anticoagulant therapy at periodic intervals, such as annually 1, 2
- The American College of Cardiology suggests that apixaban offers the advantage of reduced-intensity dosing for patients on indefinite anticoagulation whose VTE occurred ≥6 months ago 6
- For patients with DVT who require percutaneous coronary intervention (PCI), the American College of Cardiology recommends reassessing the recommended duration of anticoagulant therapy 6, 7
Cautions and Contraindications
- The National Comprehensive Cancer Network recommends avoiding apixaban in patients with severe renal impairment (creatinine clearance <15 mL/min) or significant hepatic impairment 8, 9
- The American College of Chest Physicians recommends carefully evaluating the risk-benefit ratio of extended therapy in patients with high bleeding risk 1, 3
