Switching from Fluoxetine to Duloxetine

Introduction to Switching Protocol

  • The American College of Cardiology recommends starting duloxetine 30 mg daily while simultaneously reducing fluoxetine to half dose, then discontinue fluoxetine after one week and increase duloxetine to 60 mg daily 1
  • The recommended cross-taper protocol involves reducing fluoxetine from current dose to half dose and starting duloxetine 30 mg once daily simultaneously, with monitoring for nausea, and then discontinuing fluoxetine completely and increasing duloxetine to 60 mg once daily after one week 1

Critical Monitoring Parameters

  • Cardiovascular monitoring is essential because duloxetine can cause modest hypertension and pulse elevation, with checking of blood pressure and pulse at each follow-up visit, particularly during the first few weeks of duloxetine initiation 1, 2
  • Assess for adverse effects at each visit, particularly nausea, headache, and changes in blood pressure 1, 2, 3

Special Populations Requiring Modified Approach

  • Elderly or frail patients should consider starting duloxetine at 20 mg with a slower cross-taper schedule, using smaller dose increments and longer observation periods 1, 2
  • Duloxetine is contraindicated if creatinine clearance <30 mL/min, and dosage adjustment may be needed in kidney disease 1, 2

Target Dosing and Titration

  • The standard therapeutic dose of duloxetine is 60 mg once daily, with a maximum dose of 120 mg daily if clinically indicated after adequate trial at 60 mg 1
  • Duloxetine must be taken daily, not as needed 3

Common Pitfalls to Avoid

  • Do not abruptly stop fluoxetine before starting duloxetine, as the cross-taper approach maintains therapeutic coverage and provides the smoothest transition 1
  • Do not discontinue duloxetine abruptly if it needs to be stopped later, as duloxetine requires gradual tapering over at least 2-4 weeks for patients treated longer than 3 weeks 3
