Salbutamol Administration Guidelines for Acute Asthma Exacerbations
Initial Treatment for Acute Exacerbations
- The American Academy of Allergy, Asthma, and Immunology recommends administering salbutamol every 20 minutes for 3 doses initially, then every 1-4 hours as needed based on clinical response for acute asthma exacerbations 1, 2
- For nebulizer administration, a dose of 0.15 mg/kg (minimum dose 2.5 mg) every 20 minutes for 3 doses, then 0.15-0.3 mg/kg every 1-4 hours as needed is recommended 1, 2
- The use of a metered-dose inhaler (MDI) with a spacer is as effective as nebulized therapy with proper technique and coaching 1
Severe Exacerbations
- For severe exacerbations, nebulized salbutamol plus ipratropium, hourly or continuous, is recommended 1
- Continuous nebulization in severe cases is recommended at a dose of 0.5 mg/kg/hour 2
- For optimal delivery, dilute aerosols to a minimum of 3 mL at a gas flow of 6-8 L/min 1, 2
Maintenance Dosing After Initial Treatment
- After the first hour of treatment, adjust frequency based on severity and response, with moderate exacerbations requiring administration every 60 minutes and severe exacerbations requiring administration hourly or continuously 1, 3
- Continue treatment for 1-3 hours, provided there is improvement, and make admission decisions within 4 hours 1
Discharge Recommendations
- The American Academy of Allergy, Asthma, and Immunology recommends continuing treatment with inhaled short-acting beta-agonist (SABA) as needed, continuing the course of oral systemic corticosteroid if prescribed, and considering initiation of an inhaled corticosteroid (ICS) 1
- Patient education should include medication review, inhaler technique, action plan review, and scheduling a follow-up appointment with the primary care provider within 1-4 weeks 1
