Management of Adderall Dosage Adjustment for Inadequate Symptom Control

Assessment and Dosing Considerations

  • The patient's report of inadequate symptom control on current 10mg IR dose indicates a need for dosage adjustment, especially with previous effective dosing at 30mg IR 1, 2
  • If 20mg XR is insufficient after 1 week, increase by 5-10mg increments weekly 1
  • Titrate up to a maximum of 40-50mg daily as needed, based on symptom control and side effect profile 3
  • Given the patient's previous effective dose was 30mg IR daily, they may ultimately require Adderall XR 30mg once daily 2, 3

Monitoring Parameters

  • Evaluate effectiveness using standardized ADHD rating scales at each dose adjustment 3
  • Monitor for common side effects including decreased appetite, insomnia, irritability, and cardiovascular effects (blood pressure, heart rate) 1, 2
  • Assess weight, height (if applicable), blood pressure and pulse at baseline and follow-up visits 3

Important Considerations

  • If switching from IR to XR doesn't provide adequate symptom control even at higher doses, consider alternative ADHD medications such as methylphenidate formulations or non-stimulants 4, 5

Caution

  • Document clearly if doses higher than standard are required, noting previous effective dosing history 3
