Indications for Percutaneous Endoscopic Gastrostomy (PEG) Tube

Primary Indications

  • PEG feeding should be considered when a patient's nutritional intake is expected to be inadequate for a period exceeding 2-3 weeks, with the primary goal of improving or maintaining quality of life, according to Clinical Nutrition guidelines 1, 2
  • Dysphagia following cerebrovascular stroke or craniocerebral trauma is a primary indication for PEG tube placement 2
  • Cerebral tumors, bulbar paralysis, and cerebral palsy are also indications for PEG tube placement 2
  • Parkinson's disease patients may require PEG tube placement, with special consideration given to medication-nutrition interactions 2, 3
  • Persistent vegetative state or slow recovering consciousness is an indication for PEG tube placement 4

Oncological Disorders

  • Stenosing tumors in the ear, nose, and throat region, and upper gastrointestinal tract tumors are indications for PEG tube placement 2
  • Patients undergoing chemotherapy or radiotherapy with poor nutritional status may require PEG tube placement 2, 4

Other Clinical Conditions

  • Wasting in AIDS, short bowel syndrome, reconstructive facial surgery, prolonged coma, polytrauma, Crohn's disease, cystic fibrosis, and chronic renal failure are indications for PEG tube placement 2
  • Congenital abnormalities, such as tracheo-esophageal fistula, and palliative drainage in chronic gastrointestinal stenosis or ileus are also indications for PEG tube placement 2
  • Dementia patients may be considered for PEG tube placement, but with careful consideration, as it is not usually appropriate for advanced dementia 5

Decision-Making Algorithm

  • Assess nutritional inadequacy and determine if the patient will be unable to meet nutritional requirements orally for >2-3 weeks, according to Clinical Nutrition guidelines 1
  • Try oral supplementation first, and attempt special nutritional drinks and swallowing therapy before considering PEG 2
  • Evaluate prognosis and quality of life, and consider timing, as early PEG placement is recommended to prevent significant weight loss 1, 4

Important Considerations

  • PEG placement should be for medical reasons, not administrative convenience or as a substitute for good nursing care, according to Clinical Nutrition guidelines 2
  • PEG is generally not appropriate for patients with advanced cancer, end-stage diseases, or advanced dementia 5
  • Early consideration of PEG is important, as patients typically lose approximately 12kg of body weight in the 3 months before PEG placement is considered 4
  • PEG tubes can remain in place for many years (>10 years) with proper care and do not require routine replacement 6

Indications for PEG Tube Placement

Primary Clinical Indications

  • The American Gastroenterological Association recommends PEG tube placement for patients with stroke and dysphagia, as early placement is highly effective and allows for parallel swallowing rehabilitation 7, 8, 9
  • The American Academy of Neurology suggests that patients with motor neuron disease or ALS should have PEG placement early, before pulmonary function deteriorates, with a vital capacity of more than 50% predicted 7
  • The American College of Gastroenterology recommends PEG tube placement for patients with head and neck cancers, as it can be placed before or during chemotherapy or radiotherapy to prevent weight loss 9, 10
  • The European Society for Clinical Nutrition and Metabolism recommends PEG tube placement for patients with esophageal obstruction, as it provides nutrition while an esophageal stent allows for swallowing of saliva for quality of life 7, 8

Decision-Making Algorithm

  • The American Society for Parenteral and Enteral Nutrition recommends assessing the duration and reversibility of inadequate intake, and proceeding to PEG placement if expected inadequate intake is more than 2-3 weeks 7
  • The European Society for Clinical Nutrition and Metabolism suggests that patients with a good prognosis and reversible condition have a strong indication for PEG placement 7, 10

Critical Timing Consideration

  • The American Gastroenterological Association notes that patients typically lose around 12 kg in the 3 months before PEG placement is considered, and this weight loss can rarely be fully reversed even with PEG feeding, highlighting the importance of early consideration for PEG placement 9

Absolute Contraindications

  • The American College of Gastroenterology states that coagulopathy, anatomical issues, psychiatric conditions, and prognostic concerns are absolute contraindications for PEG placement 7

Relative Contraindications

  • The European Society for Clinical Nutrition and Metabolism recommends that mild-to-moderate ascites, ventriculoperitoneal shunt, peritoneal dialysis, and pregnancy can be managed safely with experienced operators, but are relative contraindications for PEG placement 7
